All new SPA submissions must now be submitted through eProtocol.



Written Agreements

  • Written Agreements must still be submitted in hard copy with original signature to SPA.

Vendor Services Agreement

Submit two signed copies of the agreement if the project meets the Vendor Services Agreement Criteria.

Purchased Services Agreement

Submit two signed copies of the agreement if the project meets the Purchased Services Agreement Criteria.

Humanitarian Use Device (HUD) Agreement

Submit two signed copies of the agreement if using a Humanitarian Use Device (HUD).



Project Change Form

  • For SPA Amendment's to studies approved prior to eProtocol implementation (11/9/2009), please complete the Project Change Form.
