Guidelines for hospice admission
- The diagnosis of a life limiting illness. Medicare requires a prognosis of six months or less if the disease runs its natural course.
- Patient desires hospice care.
- Patient lives in the 28 county service area.
- Patient has a primary contact person who will be responsible for decision making if the patients is no longer able.
