Guidelines for Admission to Allina Home Care
- Signed physician's order
- Requires skilled nursing care, wound care, medication management and/or therapies to rehabilitate or maintain function, or for safety issues
- Patient is home-bound or experiences a very "taxing effort" to get out to the doctor
- Patient may be receiving TPN, chemotherapy, radiation, dialysis, tranfusions, IVs or wound care
- Patient needs teaching of a disease process that has caused recurrent hospitalizations and medication management
